Checklist Follow Up and COVID Illness Reporting

THANK YOU to those Members who have completed the OSSTF D18 ESSP/ECE Health and Safety Checklist for COVID-19 Issues. Your answers provide valuable information and insight into the daily challenges and concerns of dealing with COVID - 19 within an already demanding work environment. Please continue to submit your responses: 1. via online methods, or 2. print and send the completed form via the UG intra-courier mailbag (available at all schools; ask the Office Coordinator) to: D18 OSSTF Office, Elora, ON, and Attention: Julie Wagner. Employee Incident Report It is imperative that members complete the online Employee Incident Report when a workplace accident/incident (slip, fall, kick, bite, scratch, etc.) occurs. You may or may not have required first aid or any other medical attention at the time, but it is in your best interest to report all accidents in case of complications later. When completing the form, check the FIRST AID box as most injuries require basic first aid (e.g. cleaning & disinfecting of scratch/bite, ice pack to bumps/sprains, etc.).

On the “Details” page of the Employee Incident Report, check “I give permission for a copy of this report to go to my UNION President”. This ensures that the union is aware of the incident and can follow up and provide additional supports, if required.
